How to Apply for the Canadian Dental Care Plan (CDCP): Step-by-Step Guide

If you’re looking to apply for the CDCP in 2025, here’s a simple step-by-step guide to help you navigate the process.
Step 1: Determine Your Eligibility
Before applying, ensure you meet the following requirements:
- You are a Canadian resident for tax purposes.
- You filed your 2024 income tax return.
- You do not have access to private dental insurance (including through work or a family member).
- Your adjusted family net income is less than $90,000.
Phased Application Dates:
- May 1, 2025: Individuals aged 55 to 64
- May 15, 2025: Individuals aged 18 to 34
- May 29, 2025: Individuals aged 35 to 54
Eligible applicants may begin receiving coverage as early as June 1, 2025, depending on the date their application is received.
Step 2: Gather the Necessary Documents
Before starting your application, collect the following:
- Social Insurance Number (SIN)
- Date of birth
- Full legal name
- Home and mailing addresses
- Information on any dental benefits you receive through government programs (like ODSP, Ontario Works, or Healthy Smiles Ontario)
- Your 2024 Notice of Assessment from the CRA
Applying for a child? You’ll need their full name and date of birth if they don’t have a Social Insurance Number (SIN).
Step 3: Submit Your Application
The most efficient way to apply is online:
- Visit canada.ca/dental.
- Sign in or create a My Service Canada Account (MSCA).
- Fill out the application form accurately.
- Upload digital copies of your documents.
- Review and submit your application.
Prefer to apply by phone? Call 1-833-537-4342 to use the automated phone service. You can also visit your nearest Service Canada location for in-person help.
Step 4: Receive Your Confirmation
Once your application is approved, Sun Life (the CDCP administrator) will send you a welcome package within 8–12 weeks, which includes:
- Your CDCP Member Card
- Your Coverage Start Date
Step 5: Start Using Your Coverage
With your CDCP plan active, you can now enjoy your dental benefits:
- Use Sun Life’s provider directory to find participating dentists.
- Confirm that your selected dentist accepts CDCP and offers direct billing.
